Project Description:
The State of Vermont supports Vermont children and youth with disabilities to successfully transition to adulthood and achieve competitive integrated employment as part of a federal grant from the Disability Innovation Fund - Pathways to Partnerships (CFDA Number: 84.421E) led by the United States Department of Education, Rehabilitation Services Administration. The lead applicant is the Division for the Blind and Visually Impaired (DBVI), within Vermont's Agency of Human Services, Department of Disabilities, Aging, and Independent Living (DAIL).
The Center on Disability and Community Inclusion (CDCI) at the University of Vermont (UVM) proposes a scope of work to conduct the required evaluation of the larger project (Years 1 through 5) and to provide training and consultation in transition supports (Years 2 through 5).
