Project Description:
This project aims to address the need for a whole, person-centered, coordinated approach to clinical and community-based supports and services for individuals with intellectual and / or developmental disabilities and co-occurring mental and /or behavioral health conditions. There are multiple goals and objectives. OH-The Nisonger Center is leading the evaluation process.
