Children's Resource Center-South (CRC-South)
Project Description:
CRC-South provides direct support and consultation to professionals and families of children and youth with special health care needs ages 0 to 21, in the southern public health region of Wisconsin. The system of care and supports available to CYSHCN and their families is complicated. Families and professionals are given information and referrals in order to connect to needed resources. Another key activity is strengthening the existing system of care for CYSHCN through stakeholder collaboration and community outreach. Family, youth, and community voice is included in CRC-South program planning and improvement activities. CRC-South is 1 of 5 Children's Resource Center's in the state of Wisconsin that work together through the Department of Health Services as part of Wisconsin Wayfinder, Children's Resource Network. The expected outcome of CRC-S is that children and youth with special health care needs and their families experience timely, nearby, uncomplicated, coordinated supports to live and thrive.
