Project Description:
The Wyoming Department of Health (WDH), Division of Healthcare Financing (DHCF), Home and Community Based Services (HCBS) Section (Agency) supports over twenty­ five hundred (2,500) participants through three (3) HCBS Waivers, the Comprehensive and Supports Waivers, collectively referred to as the Developmental Disabilities (DD) Waivers and the Community Choices Waiver (CCW). The Centers for Medicare and Medicaid Services (CMS) requires the Agency to conduct initial and annual evaluations of all HCBS waiver participants to determine if individuals require institutional level of care. The Agency is currently using the Long Term 101 and 104 (LT-101 and LT-104) assessments, developed by WDH, to determine level of care for individuals who are emolling in the Waivers, or entering into Skilled Nursing Facilities (SNFs) or Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID), collectively referred to as Nursing Care Facilities (NCFs) for the purposes of this document. These assessments are not standardized and have not been validated, making WDH vulnerable to federal compliance violations. WIND is exploring the option of using the Functional Assessment of Standardized Items (F ASI), identified by WDH as an assessment tool that may meet its primary needs in determining institutional level of care and resource allocation for HCBS waiver participants and NCF Residents. The FASI field test builds on more than a decade of work that CMS undertook to develop standardized, interoperable assessment items and may eliminate the need for multiple initial level of care assessments for individuals and alleviate some of the administrative burden involved in developing plans of care for waiver participants. During the course of the project, WIND shall develop and implement a comprehensive exploration of the feasibility and impact of replacing the current, homegrown assessments with FASI.
