Project Description:
The purpose of the PR Family-to-Family Center (PR-F2FC) is to provide information, education, technical assistance, and peer support to families of children and youth with special health care needs, IDD and the professionals who serve them. Families of children and youth with special health care needs and IDD need to know how to navigate the healthcare system and need to be confident in how to collaborate with their healthcare providers. Because of the implementation of the proposed project, families of CYSHCN/IDD will participate more actively in the planning and the decision making related to treatments and services available at the community for their children. The focus of this F2F HIC of Puerto Rico will be to offer guidance, support, information and empower families of CYSHCN/IDD. It will be an accessible center for families to receive the information they need regarding the health services offered to their children and the guidance needed to be integrated in the decision making of the different services their children receive. The Puerto Rico F2FC will also offer a space for families and community to participate in diverse workshops offered once a month. An important aspect of the Puerto Rico F2FC will be the development and dissemination of valuable information for families of CYSHCN/IDD and professionals. Aimed to promote the development and/or replication of high-quality programs for CYSHCN/IDD and their families in and outside of PR through the development and dissemination of state-of-the-art, bilingual (English/Spanish), culturally appropriate curricula and training materials, research reports and other information related to CYSHCN/IDD in a variety of print, electronic, and audio-visual formats the Puerto Rico F2FC has the following purposes: Offer peer to peer support, valuable information, orientation and resources to families of CYSHCN/IDD. Strengthen collaboration within health system components to increase the number of families of CYSHCN/IDD who receive appropriate services. Increase the number of families and CYSHCN/IDD who are knowledgeable about the Puerto Rico healthcare system. Increase the number of professionals who are trained in family centered services to work with families of CYSHCN/IDD. Increase the number of families of CYSHCN/IDD who report been confident and competent in partnering with their childrens health care providers. Increase the number of families and professionals who are knowledgeable about the best practices, availability of resources and services in the community related to CYSHCN/IDD and how to have access to those resources.
