WI State Systems Grant for Act Early Materials in Home Visiting
Project Description:
The goal of the Wisconsin Act Early State Systems grant is to integrate LTSAE materials into two statewide programs serving parents of infants and young children in at-risk families?Department of Health Services prenatal care coordination (PNCC) and Maternal, Infant, and Early Childhood Home Visiting (MIECHI) home visiting programs contracted through the Wisconsin Department of Children and Families?so LTSAE materials and messaging will be a sustainable health education tool in a coordinated system of early identification, screening, and referral.
Objective 1.1: To design a dissemination process, by January 31, 2012, the Training Consultant and Project Team will develop the T&TA process and produce materials that support PNCC staff and home visitors in integrating LTSAE materials into their work with families, infants and young children.
Objective 1.2: To implement the dissemination process, by January 31, 2012, the Data Coordinator and Project Team will establish processes for PNCC and home visiting programs to track dissemination of LTSAE materials and timely referrals of infants and young children to early intervention services.
Objective 1.3: To evaluate project progress and outcomes, on an ongoing basis the Project Team will monitor successes and lessons learned, resulting in a final report by November 2012 that will be shared with the key stakeholders and AMCHP.
As a result of this project: 1) Both PNCC and home visiting programs routinely use and disseminate LTSAE materials in their work with parents to support optimal development of infants and young children, and 2) the WECCP and Act Early State Team are able to document (from
data in SPHERE) the impact of LTSAE dissemination through both PNCC and home visiting on referrals to early intervention. And finally, when the Project Team meets this goal, the statewide system of early
identification and referral for children and families with developmental concerns will be demonstrably closer to achieving a coordinated and integrated statewide system.
