The Wisconsin Sound Beginnings EHDI Quality Improvement Learning Collaborative is funded by a grant from the Maternal Child Health Bureau of the Health Resources and Services Administration to the Wisconsin Title V CYSHCN Program in the Wisconsin Division of Public Health. The goals include: ? Families of infants who do not pass the newborn hearing screening smoothly transition to re-screening by one month of age, to diagnosis by 3 months of age, and are enrolled in early intervention services by no later than six months of age. ? All children who are deaf or hard of hearing have a health care provider who is responsible for current health care, health care coordination, and future health planning in partnership with youth, family, and adult health care professionals. ? Providers have the core knowledge and skills to provide effective hearing health care and follow-up services. This Learning Collaborative will include four Learning Sessions over eighteen months facilitated by experts, as well as regional team meetings and activities between meetings that will enable participants to pilot and implement ideas for improvement. The expectation is that Collaborative teams share their improvement experiences and results with each other, thus, ?everyone learns, everyone teaches.? Following the Learning Collaborative, the Wisconsin Sound Beginnings Program will disseminate the lessons and tools and spread successful changes within other areas of the state and the MCHB.
