Supporting the Transition of Medicaid Beneficiaries with Disabilities into Managed Care in Michigan

DDI conducted a formative and summative evaluation of the project. This process of evaluation began by conducting a literature review that highlights Medicaid beneficiaries in transition to a managed care system. This search of best practice models focused on outcomes-based interventions that included participatory evaluation. DDI examined available assessment and care planning instruments that captured the current health status and care needs of Medicaid beneficiaries who are SSI-eligible. Following the selection of an assessment and care planning instrument, DDI consulted with persons with disabilities, advocates, and other professionals to collect input on how to best capture the current health status and care needs of Michigan's Medicaid beneficiaries who are SSI-eligible. A qualitative report detailing the findings of this task was composed and consists of a number of focus groups gathered across the state asking individuals to share their current experience and highlight opportunities to improve their Medicaid participation. DDI administered the pre-survey, post-survey, and final survey to each participants in the Living Well with a Disability curriculum. Execution of data collection for the pre-test and post-survey occurred at each implementation site. The follow-up surveys for individuals who completed the program were conducted 3 to 6 months following the conclusion of the training. This data was captured via the phone, mail, or other preferred method of data collection as indicated by the respondent. Participants were asked to complete a survey responding to the Managed Care presentation the respondents received during the training.
