Aging and Disability Resource Center Evaluation
Project Description:
This project is to develop and evaluate the effectiveness of an Aging and Disability Resource Center (ADRD). ADRD demonostration sites will be the community's single point of entry for individuals and their families seeking information and assistance in accessing long-term care services and supports. Illinois proposes centers at two sites to serve individuals with developmental and long-term disabilities and older adults experienceing age-related changes who require long-term supports and services, including individuals seeking private pay as well as public services. The Rockford Illinois center will serve older individuals and individuals with developmental disabilities and the Decatur center will serve older people and individuals with physical disabilities.
The ADRC demonstration will establish the foundation for streamlined access to long term support services for the elderly and for people with developmental and physical disabilities in Illinois. Project objectives are to:
-Maximize the ability of the elderly and persons with disabilitie to make informed choices and to meet their long-term care needs.
-Assist family caregivers in planning for the future needs of relatives with physical and developmental disabilities.
-Improve the coordination between state and community agencies that provide long-term care services in Rockford and Decatur.
-Develop a Management Information System to track clients through the service system and provide data on service utilization and costs.
-Reduce the number of nursing home and institutional placements.
The Aging Studies Porgram in the Department of Disability and Human Development at the University of Illinois at Chicago will evaluate the outcomes of this project.
