Project Description:
The primary goal of the Supporting Families in Pediatric Practices (SFPP) program is to promote and support the optimal development of young children (birth to 5 years of age) who receive care from the Children's Hospital of Michigan Pediatric Mobile Team (PMT). The operational goals and objectives of the SFPP program focus on four key aspects of child health and development, including developmental skills, health, nurturing relationships, and basic needs. Goal 1: Children will achieve developmental milestones in all domains including motor, language, cognitive and social-emotional skills. Goal 2: Children will achieve optimal health. Goal 3: Children will experience nurturing caregiver relationships. Goal 4: Children will have their basic needs met (e.g., food, clothing, housing, etc.). The program goals and objectives grew out of the lessons learned and needs identified by the Children's Hospital of Michigan Early On Collaborative Project. One of the key lessons learned was that parents need additional sources of support and information about early childhood development and parenting skills. These resources need to be widely accessible and available to parents in settings that are non-threatening. The SFPP program targets families with children who receive care from the PMT. The PMT serves communities in Detroit with high rates of poverty, which places those families at risk for limited access to health care and marginal parenting practices. By integrating developmental education and support services into the PMT health care services, the SFPP program will reach high-risk families at "teachable moments" when they are seeking health care for their children. The SFPP program goals and objectives have been designed to address key aspects of child health and development as well as parenting education and support in a family-centered and accessible manner.
