Project Description:
The Nursing Facility Transition Grant is a Center for Medicaid and Medicare System's Change Grant given to the Alaska Department of Health and Social Services, Division of Senior and Disabilities Services. This grant underwrites an effort within the Division of Senior and Disabilities Services to help individuals in nursing facilities and long-term care facilities live more independent in a community of their choice. Funds are disbursed to cover transition costs associated with moving to an independent living situation. Since the number of beds in nursing facilities across Alaska are not filled to capacity, individuals on Medicaid who move from a nursing facility to an independent or assisted living environment represent a considerable cost saving to the state Medicaid system. Health and welfare data on transitioned individuals has indicated a relatively high level of satisfaction with the program.
