Project Description:
This proposal will address the public health problem described in the background section of the notice of funding opportunity (NOFO) by providing the data needed to 1) conduct population-based surveillance of CHD to describe health outcomes including health equity, with the goal of identifying opportunities to improve the health of all people living with CHD and 2) develop and refine a machine learning model that improves accuracy of CHD case identification to enhance public health surveillance efforts. APS is well-positioned to conduct active case-finding and data collection for CHDs in individuals 0-64 years of age through existing agreements with health care and administrative organizations. Our surveillance system has maintained CHD as a surveillance population since 2002 due to the significant loss to follow-up occurring in this at-risk group. We propose to capitalize on numerous existing relationships and new collaborations and describe the approaches to create: (OC1) Improved knowledge about characteristics of individuals with CHD from multiple data sources (OC2) Improved understanding in CHD of: age-specific mortality; healthcare use, comorbidities, and outcomes; equity, racial, ethnic, or socioeconomic patterns in healthcare use and outcomes ; respiratory viruses (including COVID-19, influenza, and RSV) and immunizations; healthcare use during COVID-19 pandemic; the potential impact of state and national healthcare policies (OC3) Increased awareness among stakeholders of nationwide CHD issues (OC4) Improved understanding of healthcare utilization, comorbidities and other outcomes (OC5) Improved understanding of machine learning algorithms and models in surveillance
