Project Description:
Corazon de La Familia Infant-Child Intervention Program is a home-based intervention program for birth to five-year-olds and their families. The program is designed to provide family support, information, education, modeling, developmental guidance, behavioral interventions, mental health services, and linkages to other community resources. Funding for services is provided by the Los Angeles County Department of Mental Health and San Gabriel/Pomona Regional Center. Program staff has specialty training in working with families of infants, toddlers and preschoolers who are experiencing multiple problems and stressors. Particular program expertise exists in the provision of services to very young children with special health care needs. In-home services are designed to meet family needs, with intensity and duration of services dependent upon levels of identified need. Screenings are conducted in the home for problems in the areas of child behavior, cognition, health, nutrition, and social-emotional development, in addition to home and environmental safety. Parent education and anticipatory guidance is provided only after the staff determines need, with families as partners. The approach is relationship-based; services are collaborative, not didactic. Short-term crisis intervention can also be provided, with linkages to appropriate resources for intervention. Coordination of health care occurs with local clinics or the family pediatrician to assure a medical home for all children served. Program staff work to coordinate the delivery of services so that families are not overwhelmed by multiple providers and multiple and conflicting appointments. All services are designed to be child-centered, family focused and culturally appropriate.
