Project Description:

PHG is a community-based research program with a vision in which families of grandparents raising grandchildren are empowered, connected and thriving. PHG empowers grandparents and capitalizes on their strengths to bring about positive change. This core belief has informed strengths-based case management approaches to providing individualized support and services to families through our one year intervention program.
