Project Description:
The purpose of this project is to address barriers to healthcare access facing underserved youth who live in the City of Rochester and attend RCSD schools by promoting promote prevention, early identification, assessment, referral, and treatment for medical, developmental and behavioral health concerns; (b) leverage and build upon existing models of school-based collaboration and primary care-based integrated behavioral health; and (c) prioritize a student-centered and family-focused model with point-of-care access in schools to minimize disruption to school attendance and reduce barriers to access.
