TRACI: Pilot Site for Special Olympics/Lakeshore Foundation: Implementation of the Diabetes Prevention Program
Project Description:
The scope of work is to serve as pilot site for the Lakeshore and Special Olympics' implementation of the Diabetes Prevention Program (DPP) adapted to the needs of individuals with intellectual and developmental disabilities (IDD). We understand that this program "...is designed to help people with intellectual and developmental disabilities (IDD) who are at risk of developing type 2 diabetes" and that the program "...is based on the CDC Diabetes Prevention Program with adaptations to make the content more accessible for people with IDD and people with low health literacy." The work will be carried out by The University of Montana Rural Institute for Inclusive Communities, in partnership with Summit Independent living, the Montana Department of Public Health and Human Services, Missoula Parks and Recreation, and the Missoula City and County Health Department. The University of Montana Rural Institute for Inclusive Communities and Dr. Meg Traci will coordinate the project and team to serve as a pilot site and perform the scope of work outlined in the RFA, including: 1) Identify and train lifestyle coaches to facilitate the program in Year 1, the preparation and training phase (May-June 2025) and 2) Work with Summit Independent Living to recruit and enroll 10-15 participants per pilot site who meet he pre-diabetes risk test criteria (https://doihaveprediabetes.org/take-the-risk-test#/), with a minimum of 50% of participants with IDD; identify and train lifestyle coaches who will facilitate the program; implement the A Healthier You: Preventing Type 2 Diabetes program with additional inclusive curriculum support; and evaluate and monitor program outcomes and effectiveness through structured assessment and feedback through Year 2, the Program Implementation and Evaluation Phase (August 2025-July 2026).
