Project Description:

The Arizona Health Care Transition Performance Quality Improvement program was developed by the Arizona Research Investigating Disability and Disorders Lab in the Division of Pediatric Cardiology at the University of Arizona in collaboration with the Arizona Department of Health Services, Office of Children Youth with Special Health Care Needs to address the Arizona Title V Block Grant Program targeted performance measure.
