Project Description:

To support Options Case Management, LLC to build the capacity of case managers in the foundational knowledge and skills related to person-centered practices using the Charting the LifeCourse framework. It also aims to develop the capacity of leadership and supervisors to provide ongoing support to case managers as they implement these skills in their day-to-day work.
