Project Description:
The goal of this project is to identify and organize family support resources, with particular emphasis on behavioral health treatment and sustainable recovery (including social determinants of health). Building
on a landscape analysis already conducted by the Action Network, a dedicated staff person at IHD will compile all levels of existing programs and services targeting these families. IHD will work closely with the Action Network and its health access workgroup to determine how the resources and information will be made accessible to families, social workers, community professionals, and medical professionals statewide.
Year 1: IHD will primarily focus on compiling existing resources and updating existing resource directories from various organizations and programs. The process will include meeting with these family support programs one-on-one and emailing/calling these programs to verify the program's catchment area at the zip code level, target population, eligibility, and services provided.
Years 2-3: IHD will identify a reliable platform to host the resource hub and will work with appropriate experts to customize the design, function, and appearance of the platform for this project. The resources, information, and referral processes will be integrated into the platform and tested with the Action Network partners before being launched publicly. Potential platforms include Parent Link, DESE's new resource app for families, Simply Strategy's resource map, and others used by partners such as Missouri Family-to-Family. A dissemination plan will be shared and implemented to ensure that all audiences are aware of the resource and know how to use it.
Years 4-5: IHD will work with MHAP and MO F2F to maintain and refine the resource database and implement an evaluation plan to track the outcome and impact. Anticipated outcomes include professionals' satisfaction and self-efficacy in providing referral and linkage, and families' feedback on how the database and the hub benefited them and their families. A mixed method evaluation will be employed including a satisfaction survey every 6 months, and interviews or focus groups to understand the utilization of it, storytelling via written format or recordings from professionals and families to demonstrate the impact.
