Project Description:
To support the development and implementation of the ENTRY TRAC framework—a coordinated approach that incorporates existing tools and resources to assist families and individuals across all life domains. This effort will focus on transition readiness, serving as a key point of contact and support for youth and young adults ages 13–26. The Individual Advocacy and Family Support team will serve as a referral and support hub for families engaged with YES and other partner systems.
