Project Description:
The IOD provides services in this agreement to the NH Division of Long Term Supports and Services, ensuring technical assistance and training which promotes best practices in community services for individuals with developmental disabilities and their families. Key activities include: 1. Providing technical support and assistance for ongoing monitoring of compliance with Home and Community Based Services (HCBS} Settings expectations requirements 2. Partnering ·with lntellectability, in order to maintain, enhance and utilize their platform, in collaboration with the Department, to ensure oversight of quality waiver services, person-centered planning, and critical incident management reporting. 3. Developing and disseminating information and publications relevant to persons with disabilities and their families, to individuals, groups, associations, and organizations, statewide, nationally, and internationally. 4. Facilitating the implementation of New Hampshire's Charting the LifeCourse (CtLC) Framework with No Wrong Door (NWD) partner agencies 5. Managing ongoing access to NH Person-Centered Case Management and Service Coordination training modules (CMAT) through the hosting and administration of the ·Canvas Learning Management System (CLMS), including content development of one (1) module per year or updates to current modules as necessary.
