UCEDD faculty have joined the Pharmacy-Led Transition of Care Intervention to Address System-Level Barriers and Improve Medication Adherence in Socioeconomically Disadvantaged Populations, a multi-institutional effort funded by a R01 grant from the National Institute on Minority Health and Health Disparities (NIMHD). The goal is to test a pharmacy-led intervention during hospital-to-community care transitions to improve medication adherence and health outcomes for socioeconomically disadvantaged, chronically ill adults in Tennessee. This model builds on earlier SafeMed care transitions research, which examined medication nonadherence and care gaps among Medicaid and Medicare "super-utilizers." Dr. Carolyn Graff as co-PI is on this Project for 15% FTE. $3.5 million R01 grant 5-year duration
