Project Description:

The LifeCourse Nexus will enhance person-centered approaches through utilization of the Charting the LifeCourse (CtLC) framework by provide training, technical assistance, and implementation support to Oklahoma Department of Human Services (OK-DDS) leadership and staff. Activities will create replicable and scalable models of specific policies, practices, and procedures that implement or integrate the Charting the LifeCourse framework. Foundational knowledge and skills will be developed to build the capacity for initial implementation strategies, with shared learning and collaboration facilitated to provide ongoing implementation support and promote ongoing, continuous improvement.
