LifeCourse Pathways is an innovative program focused on supporting teens and young adults with intellectual and developmental disabilities (IDD) and their families as they transition through healthcare systems and explore supported decision-making as an alternative to guardianship. This initiative is designed to provide a holistic support structure for individuals, families, and professionals, ensuring a smooth and empowering transition into adulthood. The program offers individuals direct coaching, helping them build personalized healthcare transition plans while promoting autonomy and self-determination. Families receive tailored guidance and resources to help them support their loved ones in making informed healthcare choices without the need for restrictive guardianship. In addition, the initiative educates professionals—including healthcare providers, educators, support coordinators and social workers—on how to apply the Charting the LifeCourse (CtLC) framework to best assist individuals with IDD in making informed healthcare and life decisions. LifeCourse Pathways prioritizes collaboration between individuals, their support networks, and professionals, fostering a person-centered approach to healthcare and decision-making. While the program is currently focused on teens and young adults, it will expand to serve individuals at all life stages, ensuring that everyone has access to the tools and support they need to thrive and maintain autonomy.
