Pennsylvania is committed to ensuring that all individuals with disabilities—regardless of age or system involvement—experience a seamless, person-centered planning process that empowers them to pursue their own aspirations in education, employment, and community life. This initiative is driven by the belief that true support means enabling choice, promoting equity, and centering the individual—not merely delivering services.
To bring this vision to life, Pennsylvania is embedding the Charting the LifeCourse (CtLC) framework system-wide to foster consistency and shared understanding. The state is investing in sustainable capacity through robust training models and learning cohorts, aligning policies and practices across agencies like OVR, PDE, and ODP, and engaging individuals with lived experience from the outset. With a focus on interagency collaboration and inclusive community partnerships, Pennsylvania is equipping stakeholders with shared tools and language to ensure a unified, equity-driven approach that places individuals and families at the heart of planning.
