Project Description:
This project will consist of a four-phased process to identify how Charting the LifeCourse (CtLC) can integrate into the work of organizations who provide services and supports to individuals with disabilities. Phase 1: Exploration As an initial step, the LifeCourse Nexus will conduct a Strategic Exploration Summit with organizational leadership and invited stakeholders to establish a vision and related action plan to identify opportunities for integration and implementation of the Charting the LifeCourse framework (such as how the framework may inform the person-centered planning, training, quality management, and other processes and practices at the organization). This Strategic Exploration Summit will help to identify the specific training and technical assistance focuses for phase 2, based on the key priorities and desired outcomes of the organization. Strategy and Activities: As a result of the exploratory process consulting, the organization and the LifeCourse Nexus will collaboratively: ● Ensure a foundational understanding of the key principles of the CtLC and related tools and resources, and their applicability for the specific focus area of the interested organization ● Consider opportunities to build upon and leverage how the framework can be utilized in policy, practice, and culture change ● Engage in strategic brainstorming and initial identification of organizational priorities and opportunities for integration Phase 2: Plan and Prepare Organizations who choose to integrate CtLC into their programs that support person-centeredness are strongly encouraged to have a foundational understanding of the principles and practices of the CtLC framework, as well as a strategy for ongoing capacity building and support of all who will apply/use this philosophy in their life and work. Becoming an "affiliate" of the LifeCourse Nexus provides organizations with strategic process consulting to identify and implement innovations that ensure adoption and alignment with the principles and practices of the Charting the LifeCourse (CtLC) framework. Through the outlined activities, leadership of the organization's implementation team ("champions") will enhance their understanding, identify strategies for adoption, and support/sustain the integration of CtLC into their programs. Strategy and Activities: The LifeCourse Nexus will provide strategic facilitation and process consulting to identify key outcomes and strategies for integration of the LifeCourse framework, specifically related to the key priorities identified in the Strategic Exploration Summit. Focused professional development, technical assistance and implementation support for development, piloting, and scaling of identified enhancements and innovations will be offered to "Champions" across the organization who are responsible for integrating the framework into practices and programs. Phase 3: Implementation Support To support broader learning and capacity building across the organization – and, importantly, people and families accessing services, the LifeCourse Nexus will provide initial training for all stakeholders related to the use of CtLC in the identified practices, etc. Phase 4: Sustain and Scale To support broader learning and capacity building across the organization – and, importantly, people and families accessing services, the LifeCourse Nexus will provide training and mentoring to identified team members from the Champions team, who will be equipped to facilitate ongoing learning and capacity building opportunities.
