Project Description:
Develop a family-centered model of transition service pathway coordination across education, healthcare, and vocational rehabilitation for young Minnesota adults with epilepsy and/or co-occurring intellectual and/or developmental disabilities (IDD) who have extensive support and medical needs. The project will begin by analyzing Minnesota's current service transition systems in education, healthcare, and vocational rehabilitation for youth with epilepsy and co-occurring IDD to identify the strengths and gaps in coordinating transition services. This information will be used to develop a transition service coordination model that is centered on young adults and their families. The model will then be implemented, and its impact on transition outcomes of the young adults who participate will be evaluated.
