Project Description:
This program provides an opportunity to train individuals with intellectual and/or developmental disabilities (IDD) as Peer Navigators to help other young adults with IDD with their healthcare transition and other needed services and supports as they reach adulthood.
The ultimate goal of this program is for our Peer Navigator trainees to gain the skills and knowledge they need to obtain competitive, integrated employment in fields of social services or healthcare. As part of their training, they will be directly serving others who will be assisted with their own healthcare, employment, educational, and independent living goals. Over a 2 year period, our goal is to train 16 Peer Navigators, who will each assist and support 10 individuals with IDD with healthcare and adult service transition (160 individuals total) and to collaborate with 8 new disability-serving organizations. We hope that all 16 Peer Navigator trainees will be assisted with finding competitive integrated employment.
