Medical Home Partnership Project (MHPP)The MHPP focuses on improving the systems of care for Children and Youth with Special Health Care Needs (CYSHCN) and their families. It aims to promote quality improvement and system integration around medical homes for CYSHCN through collaboration with various partners and providing technical assistance, training, and resource development.Focus Areas1.
