Eval Group TIEH TEAM SOAR
Project Description:
The TIEH TEAM SOAR initiative will utilize the SSI/SSDI Outreach, Access, and Recovery (SOAR) model to increase access to benefits, identification, treatment, and recovery for unsheltered homeless with Serious Mental Illness, Serious Emotional Disabilities (SED), or Co-occurring Disorders (COD) in the Louisville Metro area. In Years 3-5 of the grant, the goal is for the TEAM SOAR model to spread to other community mental health centers throughout the state. University of Kentucky's Human Development Institute evaluates the TEAM SOAR grant.
