Project Description:
Partnerships for Progress for Colorado is a statewide initiative designed to improve persistent barriers in the transition to employment for youth with disabilities. A key focus of this initiative is turnover-resistant partnering—creating collaborative systems that remain effective despite frequent staff changes by anchoring the process around the needs and voices of youth and their families. The Map My Transition platform empowers youth, families, and service providers with personalized information, brief trainings, and shared tools to navigate transition services more effectively.
