Project Description:

The primary objective of this project is to utilize the CtLC concepts and tools – applied in various formats – to evaluate current policy and practice of Oregon Department of Human Services Aging & People with Disabilities (ODHS APD) Case Management and provide recommendations to enhance operations and outcomes. Additionally, given the recent publication of the Home and Community Based Services (HCBS) Access Rule, this includes an assessment, roadmap, and recommendations related to the person-centered planning recommendations in that rule.
