Project Description:
Providing assistance to The Family Health Services Division of the Hawaii State Department of Health on the Pediatric Mental Health Care Access grant in conducting key informant interviews of pediatric mental health partners (state, non-profit, and private entities) to ascertain a better understanding of the mental health landscape in Hawaii.
The expected deliverables:
- Produce a comprehensive final report that includes a summary of the project, key findings, recommendation and any actionable insights.
- Create a one-page summary of the long report that can be disseminated among partners and participants.
- Draft and finalize informed consent forms for participants, ensuring transparency about the purpose, confidentiality, and voluntary nature of their participation.
Coordinated Intake and Data:
- Draft an enrollment form for provider practice contacting the warmline for Consultation with Child Adolescent Psychiatry and Coordinated intake and referral system adhering to HRSA requirements and reporting.
- Design a coordinated intake and referral form that captures essential information for assessing participant needs and making appropriate referrals.
- Design a phone call version of questionnaire of how care coordinator will ask questions on the phone to inquiring provider.
- Design an organization tracker to keep track of calls
- Establish procedures for monitoring the quality of data collection and enrollments processes, including regular review and audits
