Elevance Health is a corporate partner of the LifeCourse Nexus, the National Training and Technical Assistance Center connecting and equipping people and organizations to use the Charting the LifeCourse (CtLC) framework for planning, problem solving, and decision making. CtLC is a framework and way of thinking, which Elevance Health has chosen to engage to enhance person-centered practices and support of the family unit in LTSS programs. The key principles of Charting the Life Course align with the organization's mission of supporting individuals to plan for their lives, take control and leverage a variety of resources to reduce reliance on paid supports to ensure the person continues to be surrounded by loved ones, friends and peers that value them and to remove the barriers sometimes unintentionally created when paid services are the only supports in a person's life. This project will support Anthem Indiana with strategic facilitation and process consulting to identify key outcomes and strategies for integration of the LifeCourse framework; create awareness of the CtLC framework and related practices for all staff; and provide focused professional development, implementation coaching and technical assistance to an implementation team to support ongoing implementation, integration and capacity building for staff and users (individuals and families).
