Kansas Self-Determined Transition Taskforce: Moving Forward to Enhance Transition Outcomes
        
     
    
        
    
    
        Project Description:
        The Kansas University Center on Developmental Disabilities, in partnership with the Kansas Council on Developmental Disabilities, the Self-Advocate Coalition of Kansas, and self-advocates, families, and other stakeholders, proposes this three-year project to implement the plan developed by the Kansas Self-Determined Transition Task force (KSDTT). The goal of this project is to implement community-based projects that empower youth with intellectual and developmental disabilities (ID/DD) throughout Kansas to achieve positive adult outcomes including post-secondary education, employment, and independent living. Project objectives include continuing to engage the KS DDT partnership to: (1) conduct a landscape analysis to identify needs in unserved and/or underserved communities and ensure resources are responsive to community needs; (2) implement the KS DTT pilot project in Saline County, an underserved and rural community; (3) expand the pilot project to three additional unserved and/or underserved Kansas communities each year; (4) create accessible resources to disseminate information and findings from the pilot; (5) create materials in multiple languages to educate families on ways to engage in the transition process; and (6) develop a sustainable infrastructure for further expanding the pilot.
