Adults with disabilities can face communication challenges significantly affecting the quality of healthcare they receive, amplified by provider bias and lack of specialized training. The Holographic Augmentative and Alternative Communication (HAAC) Library will innovate provider training and competency by leveraging holographic technology and the lived experiences of people who use augmentative and alternative communication (AAC). Centering the expertise of people who use AAC, this library seeks to improve provider competency, empathy, and quality of care. Ultimately, the goal is to empower people who use AAC, challenge biases, and advance systemic change in healthcare. Project Aim. This project will develop the Holographic Augmentative and Alternative Communication (HAAC) Library, created in partnership with adults with disabilities. The Library will host video cases and supplemental training materials, compatible with holographic and traditional online viewing platforms, showcasing lived experiences of adults with DD describing their preferences and priorities related to accessing high quality healthcare. This project adopts an innovative approach to actively engaging adults with DD in the research and development process, to produce a training for healthcare and related professionals that directly address the self-identified needs of this population. Study Approach. This mixed methods study will follow the Exploration, Preparation, Implementation, and Sustainment (EPIS) framework. In Phase 1, Exploration (3 months), high-priority training topics for the HAAC Library content will be identified by engaging 3 key stakeholder groups via focus groups and online surveys: (a) 5 PWUAAC, (b) 5 caregivers of PWUAAC, and (c) pre-service and practicing healthcare and related professionals. Analysis of focus group and survey data will inform both content (healthcare training topics) and formatting (e.g., module-based trainings, case study videos). In Phase 2, Preparation (9 months), qualitative and quantitative data collected from Phase 1 will be used to create a pilot holographic recording and associated training materials (discussion prompts, related resources). A community engagement studio (CES) with key stakeholders (PWUAAC, caregivers, healthcare professionals) will be used to examine acceptability and feasibility, and to solicit feedback on recommended adaptations to the HAAC Library. CES data will be integrated into development of the remaining 9 cases for the HAAC Library and supplemental training materials. In Phase 3, Pilot Implementation, the HAAC Library will be disseminated to pre-service and practicing healthcare and related professionals (see dissemination plan below), for implementation in authentic educational and healthcare practice settings.
