Project Description:
The Nevada Aging and Disability Services Division has been the state's lead for development and implementation of No Wrong Door functions since 2005. In Nevada, the NWD system is known as Nevada Care Connection. As the state's population continues to grow at level exceeding the national average, ADSD recognizes the need to re-examine our approach to NWD implementation and increase coordination among state level partners. The goal of the project is to establish a steering committee that will serve as the NWD governing body for the ongoing administration of NWD activities, including efforts to develop a Master Plan for Aging. Nevada has established the following objectives: (1) Establish a NWD Steering Committee and corresponding infrastructure of support; (2) Conduct analysis and assessment of Nevada's existing access points, to include technology and person-centered counseling; and (3) Develop a comprehensive plan for ongoing administration and development of NWD functions. The ADSD will publish annual reports of the Nevada Care Connection efforts to document progress towards the implementation and ongoing maintenance of the NWD functions and Key Elements, as well as to increase recognition and support of the network. NCED will assist with team facilitation, research/evaluation, and information dissemintation.
