Project Description:
Progression Across Transitions in Healthcare (PATH). PATH is designed to create a care
network of adult providers that will provide a coordinated, interdisciplinary program leveraging
the use of telehealth, allowing flexibility for the patients and the providers to tap into the
network, access care, and collaborate in an interdisciplinary manner to provide comprehensive
care. The network will involve an expanding list of physicians of all specialties, as well as social
workers, dentists, therapists, and more, who are able to competently provide care to adult and adolescent patients with ASD/DD and complex medical conditions. PATH will be a new clinical
opportunity for medical subspecialities, including neurology, family medicine, psychiatry,
dental, nursing, law, and public health, that will enable patients to graduate seamlessly from
pediatric specialty care into a capable, interprofessional adult team that will manage their care
across the lifespan.
PATH will also serve as a training ground for innovative approaches to improving the quality
of healthcare delivered to adults with ASD/DD. Health care professionals, medical students,
residents and fellows will gain real-world clinical expertise by providing direct patient care
through the PATH clinic. Other LEND disciplines, such as social work and physical therapy,
will also participate.
