Project Description:
The LifeCourse Nexus will: 1. Strategic Exploration and Planning a. Provide ongoing support, process consulting, and technical assistance to design, facilitate, and evaluate the initial implementation strategic plan, with a specific focus on building capacity within KDADS to replicate, scale, and sustain the program and ensure increased standardization of PCP planning processes and tools across provider types and populations. 2. Training and Capacity Building a. Host in-person/virtual training across the state to create a foundation of understanding of Charting the LifeCourse (CtLC) across all target audiences (target population, practitioners, implementers and leaders) b. Facilitate both in-person and virtual shared learning opportunities to build the knowledge and skills of a selected Implementation Team (representing all regions) in person-centered thinking and practices c. Support for the Implementation Team to develop and carry out region specific Implementation Plans for the application of CtLC within specific elements of their programs and services d. Certify Implementers and Trainers within each region to support sustainability and growth throughout the region 3. Learning Network for Scaling and Sustaining a. Develop a statewide learning network structure based on regions and targeted implementors and members b. Facilitate regular, virtual learning opportunities for all staff to raise awareness and build skills for using CtLC in day-to-day practice.
