Project Description:

UMKC-IHD LifeCourse Nexus staff, as Subcontractor, will assist HSRI in supporting the state of New Hampshire. Specifically, the LifeCourse Nexus (at UMKC-IHD) is committed to building upon our existing work in the National Community of Practice for Supporting Families, of which New Hampshire Department of Health and Human Services is a member and whose efforts are focused on developing a core/common set of standards for person-centered practices across all bureaus. UMKC-IHD staff will utilize this existing framework (currently in development) and further collaborate with HSRI and the Department to engage stakeholders to inform the curriculum design process. UMKC-IHD LifeCourse Nexus staff will also help facilitate a multi-disciplinary development team in the design and delivery of training materials and resources for case managers—both "core competency" related curricula, as well as lessons specific to the essential functions of the differentiated roles specific to the various waivers within New Hampshire.
