Service Coordination Unlimited
Project Description:
Purpose: To support the ID Department of Service Coordination Unlimited to continue to develop policies, practices and procedures for supporting individuals with IDD and/or Autism within the context of their family through adoption and implementation of the Charting the LifeCourse framework for planning, problem-solving, and decision making. This effort will build the capacity of the Service Coordination team within this department to integrate CtLC into their day to day work as well as provide strategic facilitation, coaching, and mentoring to ensure scalability and sustainability of identified practices.
Strategy: The LifeCourse Nexus will provide focused professional development, technical assistance and implementation support to the Service Coordinators, Manager, and Director of the ID Department.
