The goal of the Achieving Health Equity for Alabamians with Disabilities (AHEAD) project is to engage healthcare practices to do the work of transforming the healthcare they deliver to make it more accessible and more effective for individuals across the lifespan living with disabilities, including intellectual, developmental, sensory, mobility, and other disabilities. The project will achieve this goal by engaging practices in a needs assessment process to describe the strengths and opportunities each practice has related to its approach to and process of providing care for individuals with disabilities. The needs assessment process will highlight areas of focus for the core functions of ongoing education of healthcare providers on disability-related topics and best practices, in addition to technical assistance and to facilitate practice change and quality improvement in the target areas informed by the needs assessment. Method Create a needs assessment. Identify, recruit, and maintain partnerships with at least 3 pediatric and 3 adult primary care practices (with a cap of 5 pediatric and 5 adult primary care practices; experience gained through this project will inform future opportunities for scaling up). Have these practices complete the needs assessment and then tailor educational content, technical assistance, and on-site assessments based on the information gathered from the needs assessment data. Additionally, based on these experiences and existing knowledge/material from nationally established resources, we will work to develop a reusable curriculum that can be distributed to future partners
