Project Description:
The Rural Institute (RI) is collaborating with Public Health Workforce Expansion funds with the Montana Council on Developmental Disabilities and Disability Rights Montana (DRM) to create and support a health and wellness outreach coordinator (HWOC) position, supervised by DRM but guided by the three DD Act partners. RI also will leverage the AT Ambassador initiative being supported by MonTech's PHWF award. (MonTECH is using its additional and related award of $80K PHWF to support an AT Ambassador initiative.) Dr. Traci and Ms. Russell, AUCD Trainee will continue its coordination role through the Montana Disability and Health Program of the Montana Aging and Disability Network calls and communications. The HWOC and AT Ambassadors will participate in those communications. The Montana Disability and Health Program (MTDH) will assist disability-related ambassadors and information coordinators, and the HWOC to promote inclusion best practices by the public health workforce in the MT Aging and Disability Network. These include Rocky Mountain Accessibility Ambassadors, MTDH Accessibility Ambassadors, AT Ambassadors, Dementia Care Coordinators, Act Early Ambassadors, etc. MTDH will develop/ disseminate a core of messages and resources to be used by the MT Aging and Disability Network.
