Family Voices Stay Covered PHE Unwinding

Project Description:

Project Overview: This project will meet the critical funding and technical assistance needs of Family-to-Family Health Information Centers related to the public health emergency unwinding. Opportunity: The goal of this project is to help families understand and address issues related to the unwinding of the public health emergency. Scope of Work: 1. Participate in an initial needs assessment survey. 2. Attend OR watch recordings of six training and technical assistance webinars provided by Family Voices. 3. Participate in one focus group in January 2024. 4. Customize materials developed and provided by Family Voices, as applicable. 5. Support families through the redetermination process and navigation of services. 6. Establish partnerships to provide education, training and technical assistance to participants/family caregivers related to the public health emergency unwinding. 7. Report activities, strategies, and materials used to outreach families. 8. Using the online reporting tool, submit monthly reports on outreach, training, and navigation outcomes as applicable to your tier of funding.
