Project Description:
This project will build capacity among stakeholders in local communities, identify
areas of need, help states plan for and address Programmatic, Policy, Systems and Environmental
Changes (PPSEs), and work with community partners to implement and evaluate those changes to
support the health of people with mobility limitation (ML) in their state. States will work with NCHPAD
to address CDC-funded joint goals of 1) identifying and addressing needs in various localities, 2)
increasing delivery of high-quality programs and services to promote health for people with mobility
limitations, and 3) providing and assessing opportunities to improve health outcomes for people with
disabilities.
This Planning and Implementation project will take place over the remainder of the CDC-funding
cycle, through July 2026, pending annual awards. NCHPAD and the Disability and Health State will
work together through a sub-contact to accomplish the following broad objectives.
1) Train stakeholders on an inclusive implementation process (NiCIP)
2) Build capacity for effective partner collaboration
3) Select sustainable, inclusive programmatic, policy, systems and environmental (PPSE)
strategies that support inclusion of people with mobility limitation
4) Contribute to health outcome data collection and coordination
