Project Description:
Ohioans with disabilities experiences a variety of health inequities and barriers to receiving high-quality healthcare. The 2021 Health Value Dashboard indicates the following disparities are present for adults with disabilities: adult depression is 3.6 times worse; adult diabetes is 2.1 times worse; and food insecurity is 3.1 times worse for Ohioans with disabilities. Additionally, the inability to see a doctor due to cost is 2.6 times worse for Ohioans with disabilities. Data from the Health Value Dashboard indicate that eliminating disparities would save 181,488 Ohioans with disabilities from delaying health care due to cost. Many of these disparities are driven by service deficits, ableism, and other forms of discrimination. There is a great need for enhancing partnerships among organizations that serve adults with disabilities, to implement policy changes that promote improved access to care and health outcomes, and to assess communities needs to identify gaps in resources and tools. The Ohio Disability and Health Partnership is a CDC-funded project that aims to migitate health barriers faced by Ohioans with mobility limitations and intellectual disability. The project aims to address six strategies: to (1) establish, expand, and enhance partnerships; (2) conduct community needs assessments and identify action steps to fill the gaps; (3) administer and evaluate training for healthcare providers on accessible preventive health care; (4) implement and evaluate a demonstration project to link adults with IDD to preventive health care and health promotion programs; (5) implement and evaluate evidence-based health promotion interventions and PSE changes and needed resources and tools; and (6) disseminate key findings and lessons learned. Some unique features of this project include a demonstration project that utilize community health workers (CHWs) to create linkages to health services, and development of education materials for future healthcare professionals. Expected outcomes of this project include the following: Strategy 1 (S1): Establish, expand, and enhance partnerships statewide. ST 1.1: Stronger engagement and coordination between recipients and key partners. IM 1.2: Broader reach among the target populations. S2: Conduct a statewide needs assessment; develop resources and tools. ST 2.1: Recipients gain knowledge about state needs, and a repository of resources and tools to meet those needs. IM 2.2: Ohio uses resources and tools to improve the health of adults with IDD and adults with ML. S3: Administer and evaluate health care provider training on accessible preventive health care. ST 3.1: Providers gain disability and health competency. IM 3.2: Providers improve efforts to deliver accessible preventive health care. S4: Implement and evaluate a demonstration project to link adults with IDD with unmet needs to preventive health care and health promotion programs in their community. ST 4.1: Participants gain knowledge about recommended preventive health care and healthy lifestyle behaviors. ST 4.2: Participants gain knowledge about addressing barriers to preventive health care. IM 4.3: Participants are appropriately linked to preventive health care, mental health care and health promotion activities. S5: Implement and evaluate evidence-based health promotion interventions, policy, system and environmental (PSE) changes. ST 5.1: Participants enroll in evidence-based health promotion interventions. IM 5.2: Participants adopt healthy behaviors. IM 5.3: Ohio experiences notable PSE changes. S6: Disseminate key findings and lessons learned. ST 6.1: Recipients learn best practices and lessons learned. IM 6.2: Health professionals and policy makers gain knowledge about ways to promote health among adults with disabilities.
