Project Description:

IOD is working with Human Services Research Institute to complete a systems assessment and gap analysis of New Hampshire's home and community-based services by providing expertise on the local community, service, and policy landscape, connections and support in asset mapping of relevant communities in New Hampshire and collaboration in support of community data collection, as well as contributing to a review of findings and recommendations.
