Project Description:
Purpose: To support the LifeSpan Respite grant partners, including the NY State Office for Aging, New York Connects, and the state respite coalition to utilize and integrate the Charting the LifeCourse framework and tools for Respite into person-centered options counseling policy, practice, and procedure in sustainable and scalable ways.
Strategy: Provide focused professional development, technical assistance, and implementation planning/support to the selected representatives from the LifeSpan Respite Grant team and the designated trainers of options counselors, who will identify and plan for specific implementation strategies for initial and ongoing training, explore opportunities for evaluation and measurement, as well as build their capacity to share the LifeCourse framework and tools with options counselors.
Activities/Deliverables:
• CtLC Overview Webinar to introduce the framework and it's application for respite planning and support to central office staff.
• CtLC Ambassador Series for up to five (5) Certified Ambassadors, who will be equipped to support the integration of the CtLC principles in existing and/or developing policies, practices, and training for their team and to share the CtLC framework with other staff and contracted providers
• Up to twenty (20) hours of technical assistance and training development, to support the New York team in developing customized curriculum for ongoing training in the use of the CtLC Respite tools
