Project Description:
Purpose: To support leadership and staff within CICOA to further adopt, operationalize, and implement person and family centered thinking and practice, and to utilize and integrate the Charting the LifeCourse framework into organizational policy, practice, and procedure to support individuals with disabilities and their families.
Strategy: Provide overview information for all staff, and focused professional development, technical assistance, and implementation support to the Leadership Team for strategic organizational change brainstorming and planning and to identified "Champions" to identify person centered practices that can be implemented or enhanced in the day-to-day work.
Activities
As a result of the activities CICOA will receive:
• One (1) in person Community Wide Event to introduce a wide range of CICOA staff to Charting the LifeCourse. The Community Wide Event is a combination of an introduction to the LifeCourse Framework and direct facilitation using LifeCourse Tools and Principles with attendees.
• One (1) half-day strategic brainstorming event following the Community Wide Event with leadership and champions to brainstorm and plan the future of LifeCourse implementation for CICOA.
• Up to Thirty (30) Certified Ambassadors, who will be equipped to integrate Charting the LifeCourse into their professional practice and introduce others to the CtLC framework.
• One (1) In Person Ambassador Summit
o In person or virtual event for up to 30 Ambassadors and additional organizational leadership team members and other staff (as appropriate) to identify priorities and strategies for ongoing implementation and organizational change
• Up to 18 hours of Implementation Support and Coaching
o Monthly 1.5-hour implementation coaching calls with Ambassadors to support ongoing learning, application, and integration of identified practices.
• Up to 12 hours technical assistance with leadership team
o Technical assistance calls to plan for and support implementing key activities and support Champions in their implementation.
