Project Description:
The New Hampshire Disability and Health Program (DHP) improves the health of people with disabilities in New Hampshire, using evidence-based strategies to increase access to preventive health care and encourage healthy behaviors. The purpose of DHP is to address health disparities or differences among adults with intellectual and developmental disabilities (IDD) and adults with mobility disabilities. DHP will work to: identify and address community needs; link adults with IDD to preventive health care and health programs in their communities; train healthcare providers; promote policy, system, and environmental changes; and share key findings and lessons learned. DHP has an Advisory Work Group of experts in public health, health care, and disability, including advocates with IDD or mobility disabilities, family members and caregivers. DHP collaborates with other state awardees, CDC, and the National Centers on Disability.
